94 APPENDIX The Application of HCM on the Basis of a Clinical Case
✔✔Transition from outpatient to inpatient with HCM
From the HIS, the HCM system receives an HL7 ADT message informing
it when Frank will be admitted to the hospital. The HCM system then automatically
»asks« the VNA via DICOM query whether data from Frank’s
previous visits is available. Existing data is transferred from the »slow«
VNA to a faster storage area. This ensures that the physician can quickly
get an overview as soon as Frank is in the hospital.
When Frank arrives at the hospital, he hands over his health insurance card, the
letter of referral from his physician, the filled-out case history form and a storage
medium with radiological images. The employee at the patient reception accesses
Frank’s information using his health insurance card in the HIS. She discovers that
Frank’s last name has changed since his last visit. He recently got married. The
employee not only creates a new case, but also changes the patient’s master data.
✔✔Master data modification with HCM
The HCM system receives the information about the name change from
the HIS via HL7 ADT message and applies it to the existing file on the fast
storage unit. The VNA is also notified via HL7 ADT message and modifies
the data as well.
Now the employee has to take care of the documents brought in by the patient:
She places the referral document and the case history form on a scanner
and starts an import tool from within the HIS that triggers a scanning process in
the patient’s context. Once scanned, the documents are directly mapped to the
appropriate
registers of the in-house filing plan. Importing the data carrier works
in a similar way.
✔✔ Import of provided data and documents with HCM
The scanner itself does not have any standard medical interfaces. Therefore,
the import tool acts as interface. It ensures that the scanned documents
can be received as PDF files. After that it accesses the registers of the inhouse
filing plan and displays them along with the patient information
from the HIS and the documents. After viewing and classifying the documents,
they are transmitted to the HCM system.
Once the patient admission formalities have been completed, Frank receives a
bracelet with the case number encoded as a barcode and is admitted to the surgical
ward.
